Chubb Personal Insurance Quoting and Binding Procedures

Completed ACORD® applications for each line of business requested can be e-mailed to PLinfo@glatfelteragency.com or faxed to (717) 849-4949. The Chubb supplemental information form below is required for home quotes.

• Quoting turnaround time is 72 business hours for most risks as long as the application is complete.
• Please provide contact information with each quote request to include your agency name, address, phone, fax and email address.
• If quote is acceptable to the customer and you would like to bind coverage, please contact our Personal Lines Department at (800) 632-1884 or e-mail your request with the effective date of the risk. Also advise if any coverage changes are desired.
• All signed applications with any applicable forms should be kept in your office for reference.
